Requirements For Students
It is incumbent upon each student to provide all required documentation in a timely manner as part of the pre-enrollment package. Students must log on to the Student Health Portal in order complete all of the requirements. Please review all of the instructions on the Student Health Portal very carefully.
Medical Hold Immunization Certificate Form
Below you will find a link to access the required Immunization Certificate Form. Please download and ensure that all required fields are completed by a physician. Once you have access to the Student Health Portal, please enter the required information and upload your form.

Registration Deadlines
The deadlines for registration are June 30th, for the fall semester and December 1, for the spring semester. You must use your Howard University Student ID number and Bison email to submit documents.
